Job Description:

• Medication Therapy Management (MTM): Reviewing patient profiles, medical charts, and lab results to assess the appropriateness, efficacy, and safety of prescribed medications. 
• Clinical Rounds Interventions: Participating in daily ward rounds with physicians to provide real-time recommendations on drug selection, therapeutic drug monitoring, and dosage adjustments (e.g., renal or hepatic dosing).
• Patient Education & Counseling: Meeting with patients and families to explain medication regimens, proper administration techniques (e.g., insulin or inhalers), and how to manage side effects. 
• Adverse Event Prevention: Reconciling medications during care transitions to prevent prescription errors, drug duplications, or dangerous interactions.
• Chronic Disease Support: Assisting in managing long-term conditions like diabetes, hypertension, and asthma through collaborative drug therapy management protocol privileges. 
• Protocol & Research Development: Creating evidence-based hospital formularies, safety guidelines, and analyzing recent clinical trials to incorporate into daily practice.
